No need to travel to an office – your Sparrow CHW will meet you wherever you’re most comfortable, including over a phone call, text, or in-person.

  • Share your story. Your Sparrow CHW will listen to you, help identify your challenges, and create a personalized care plan, just for you.
  • We won’t just help solve your current challenges; we’ll also help you uncover the root causes of your struggles and offer you recommendations to better manage them.
  • With Sparrow, you’re in charge of your future. We’ll encourage you to climb to new heights with the right team and the right plan created just for you.

Act

  • Your Sparrow CHW can help you:
    • Find and keep housing.
    • Get help paying the bills.
    • Put food on the table.
    • Get a job you want.
    • Connect with other people like you with similar hopes.
  • We also connect you with professionals to help you take care of your mind and body, including:
    • Finding out if you’re eligible for local, state, and federal benefits and programs and helping you to enroll.
    • Scheduling visits with local primary care providers (PCPs)*, specialists, mental health professionals, and pharmacists. Your Sparrow CHW can even attend your visits with you, if you’d like.
    • Giving you tools that help you better manage your medications.
    • Offering follow-up assistance after an emergency room visit or hospital stay to help you re-adjust to everyday life.
    • Reducing your stress and anxiety with specialized coaching, including therapy.
    • Assisting you with your diet and exercise goals, including meal planning.
    • Helping you manage any chronic diseases, such as diabetes or high blood pressure
    • Connecting you with peer support groups to help you through any condition.

*Already have a PCP? Great! We’ll partner with your provider to ensure you get the full range of services you need to get you on track to improved health.

Stay in Touch

  • Our work doesn’t end once you’re back on solid ground. Whenever you need us, we’re only a phone call or text message away – and we’ll check in once in a while to see how you’re doing.
  • Get exclusive text messages from the Sparrow team filled with tips, alerts about new resources, and other informative content so you can continue your journey toward self-improvement. 
  • Join a peer support group anytime. These can take many forms, including group chats, phone calls, and in-person meetings.

Do I have to pay for Sparrow’s services?

No. Sparrow is offered at no cost to you and your family as a covered benefit by your Medicaid health insurance plan.

If I need help right away, who should I call?

If you are experiencing any kind of emergency, please call 911.

Can I use Sparrow anywhere in the United States?

Right now, Sparrow is only working with members in California. Check back soon as we launch in new locations!

Who can become a Sparrow member?

As of today, you can become a Sparrow member if you are eligible for the Medi-Cal benefit of CHW services and live anywhere in California except San Luis Obispo or Santa Barbara counties. We serve members insured by any of the following medical plans:

How does Sparrow partner with clinical providers and community-based organizations?

We help extend the reach of partners, primary care providers, hospital EDs, social service agencies, and community-based organizations, closely coordinating each member’s care to ensure they have a seamless experience navigating the healthcare system and can spend their time thriving in their community.
